THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.
MedRx Infusion Clinical Pharmacy (referred to as “our” or “we”) is dedicated to maintaining the privacy of your medical information. In providing services to you, we will create and maintain records containing your identifiable medical information, referred to as protected health information (PHI). We may store, use, and disclose PHI in paper form or electronically.
The Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) restricts how your PHI may be used and disclosed. This Notice of Privacy Practices (“Notice”) provides important information about how we may use and disclose your PHI, your privacy rights under HIPAA, and our obligations concerning your PHI.
We are required by law to:

Provide you with this Notice of our legal duties and privacy practices
Abide by the terms of the Notice currently in effect
We reserve the right to change the terms of this Notice. Any changes will apply to all PHI we maintain. We will post a copy of the current Notice on our website, and you may request a copy at any time.
HIPAA allows us to use and disclose your PHI without your authorization for the following purposes. Not every possible use or disclosure is listed.
We may use and disclose your PHI to provide, coordinate, or manage your healthcare. This includes consulting with other healthcare providers and dispensing prescription medications.
We may use and disclose your PHI to bill and collect payment for services. This may include contacting your insurer, billing you directly, or seeking payment from responsible third parties.
We may use and disclose PHI to operate our business, including quality assurance, business planning, legal services, and care coordination. We may also contact you with reminders or information about health-related services.
Unless you object, we may disclose your PHI to family members or friends involved in your care or payment.
We will disclose PHI when required by federal, state, or local law.
We may disclose PHI to public health authorities or agencies such as the FDA.
PHI may be disclosed for audits, investigations, inspections, licensure, or compliance monitoring.
We may disclose PHI in response to court orders, subpoenas, or lawful processes, including special protections under 42 CFR Part 2 for substance use disorder records.
We may disclose limited PHI to law enforcement officials as permitted by law.
We may disclose PHI to prevent or lessen serious threats to health or safety.
We may disclose PHI for workers’ compensation or similar programs.
PHI may be disclosed to identify a deceased person or determine cause of death.
We may disclose PHI for organ or tissue donation and transplantation.
PHI may be used or disclosed for research purposes in compliance with federal law.
We may share PHI electronically with other healthcare providers through HIEs.
We may share PHI with business associates who are legally required to protect it.
We may use or disclose de-identified PHI for research, public health, or operations.
PHI may be disclosed to authorized government agencies when abuse is suspected.
We may disclose PHI for military or national security activities.
PHI of inmates or individuals in custody may be disclosed under certain circumstances.
HIPAA prohibits the use or disclosure of PHI to investigate, impose liability, or identify individuals for seeking or providing lawful reproductive healthcare. When required, we must obtain written attestations confirming PHI will not be used for prohibited purposes.
We will obtain written authorization for uses and disclosures not described above. Authorization is required for:

Marketing purposes
Sale of PHI
You may revoke authorization at any time by contacting us.
You have the right to:
Request confidential communications
Request restrictions on PHI use or disclosure
Access and obtain copies of your PHI
Request amendments to your PHI
Receive an accounting of disclosures
Obtain a paper copy of this Notice
File a complaint without retaliation
Requests must generally be submitted in writing, and reasonable fees may apply where permitted by law.
